Support Coordination Client
Jacob receives Level 2 Support Coordination through Sunflower Services with Sophie Edwards, who helps manage his therapies and support for physical recovery. Together, they’re working towards his goal of living independently. A true embodiment of the Sunflower vibe, Jacob brightens our office with his energy, fun, and laughter.
I’m 23 years old and love staying active—whether it’s hitting the gym, spending time at the beach, or hanging out with mates. I’m a laid-back guy with a passion for footy, and the Newcastle Knights are my team.
After contracting Streptococcus Pneumonia Meningoencephalitis in August 2023, my Support Coordinator, Sophie Edwards, successfully engaged a dedicated team of therapists to support my transition home. This team included Occupational Therapist Emily (Sakura), Physiotherapist Emma (Levaro), Speech Pathologist Kim (Engage VR), and Case Worker Zaid (Sunflower Services). More recently, Sophie has also helped me take the next step toward moving out, with the support of Sunflower’s Accommodation Manager Tess.

Ann has received ongoing support from Sunflower Services, helping her achieve key milestones in life. From securing more suitable accommodation to budgeting for social outings, her journey has been filled with growth and new experiences. A vibrant and social person with a deep love for music, especially Cher! Ann is rediscovering her love for swimming, exploring the community, and confidently enjoying life.
Ann’s journey with Sunflower Services has been life-changing. Last April, her Support Coordinator, Emma Danks, helped her secure more suitable accommodation, providing advocacy through the NDIS and DCJ housing for a great outcome.
Now, with support from Sophie Edwards, Ann stays on top of budgeting, making it easier to save for social activities. A recent highlight? A magical day at the Butterfly House with her Support Worker, Sarah.
She’s also rediscovered her love for swimming, returning to the ocean baths after 50 years to swim at sunrise. Another milestone was securing hearing aids, helping her feel safer and more connected in the community.

Level 2 Support Coordinators help participants navigate the NDIS and coordinate services. They assist individuals with complex needs by connecting them with providers, arranging supports, managing budgets, and handling NDIS communication. They ensure services work together effectively while helping participants overcome challenges, achieve goals, and build skills for greater independence.
Jake has an autism diagnosis and requires speech therapy, occupational therapy, and in-home support workers. His parents struggle to keep up with coordinating these services, ensuring funding is used correctly, and navigating the complex NDIS system. A Level 2 Support Coordinator steps in to organise everything, and engage the appropriate services.
Managing relationships with therapists, social workers, and support workers.

Ensuring appropriate funding provision and supporting efficient use of funds in line with participant goals and assistance in identifying the need for, and facilitating, NDIS plan reviews.
Level 3 Specialist Support Coordination provides expert assistance for participants with complex, high-risk needs, including severe disabilities, mental health conditions, and homelessness. Our Specialist Support Coordinators advocate for participants, help navigate barriers to essential services, and coordinate both NDIS Specialist Support Coordination and mainstream supports.
Our Level 3 Specialist Support Coordination team works closely with professionals to ensure services align, assisting with the management of complex situations and empowering participants to build long-term independence through a capacity-building approach.
Emily has a schizophrenia diagnosis and is at risk of homelessness due to unstable housing. She struggles to engage with services and has difficulty maintaining treatment. A Sunflower Services Level 3 Specialist Support Coordinator works intensively with her to find stable supported accommodation (in this case, SIL), liaise with mental health professionals, link with appropriate crisis services and ensure an intervention plan is developed and implemented to reduce or prevent the reoccurrence of crises and emergency in the future.

Specialist Support Coordinators assist participants with complex mental health conditions, psychosocial disabilities, and crisis intervention needs. Our team provides expert support to stabilise care, connect with specialists, and manage long-term support, ensuring participants receive the right NDIS and mainstream services.
NDIS Specialist Support Coordination assists participants at risk of homelessness, transitioning from hospitals or justice settings, aiming to live independently, or seeking to secure Specialist Disability Accommodation (SDA), Individualised Living Options (ILO), or Supported Independent Living (SIL).
Specialist Support Coordination manages care for participants with severe physical disabilities, degenerative conditions, or complex therapy requirements. We coordinate multiple specialists, equipment needs, and long-term health planning to ensure comprehensive support.
Level 3 Support Coordination works alongside the NDIS, Centrelink, the Justice System, Guardianship bodies, hospitals, medical professionals, schools, allied health, support services, and advocacy organisations to ensure participants receive appropriate funding, representation, assistance, and support.
A young participant with autism struggled with challenging behaviours at school, affecting both learning and social integration.
Our Level 3 Specialist Support Coordinator collaborated with the school, therapists, and family to create a tailored action plan, ensuring the right educational and therapeutic supports were in place.
With the right interventions, the participant’s confidence grew, behaviours of concern reduced, and they experienced a more positive school environment with better social connections.
A participant with severe anxiety and depression was at risk of losing their home due to difficulties managing daily tasks.
Our Level 3 Specialist Support Coordinator worked closely with the participant’s mental health team, housing support, and NDIS providers to develop a coordinated plan of action.
With immediate access to mental health services, stabilised housing, and tailored coping strategies, the participant’s well-being improved, allowing them to successfully maintain their home and tenancy.
A participant with a complex medical condition required multiple health services and frequent hospital visits but struggled to coordinate these alongside their NDIS supports.
Our Level 3 Specialist Support Coordinator facilitated communication between doctors, support workers, and NDIS providers, ensuring a streamlined approach to both health and disability support.
With a tailored plan for regular check-ins, optimised NDIS funding, and access to additional eligible supports, the participant gained greater independence and reduced stress in managing their condition.
A participant on parole struggled with compliance due to mental health challenges and limited support networks.
Our Level 3 Specialist Support Coordinator worked with their probation officer, mental health team, and key services to secure stable housing, mental health support, and structured assistance for parole check-ins.
