Jacob receives Level 2 Support Coordination through Sunflower Services with Sophie Edwards, who helps manage his therapies and support for physical recovery. Together, they’re working towards his goal of living independently. A true embodiment of the Sunflower vibe, Jacob brightens our office with his energy, fun, and laughter.
I’m 23 years old and love staying active—whether it’s hitting the gym, spending time at the beach, or hanging out with mates. I’m a laid-back guy with a passion for footy, and the Newcastle Knights are my team.
After contracting Streptococcus Pneumonia Meningoencephalitis in August 2023, my Support Coordinator, Sophie Edwards, successfully engaged a dedicated team of therapists to support my transition home. This team included Occupational Therapist Emily (Sakura), Physiotherapist Emma (Levaro), Speech Pathologist Kim (Engage VR), and Case Worker Zaid (Sunflower Services). More recently, Sophie has also helped me take the next step toward moving out, with the support of Sunflower’s Accommodation Manager Tess.
The transition has been smooth and welcoming, and the support from the Sunflower team has been incredible. My Sunflower CoS and Support Workers (shout out: Bec, Cam, and Adam), consistently go above and beyond and my Case Worker, Zaid, has been outstanding.
I couldn’t have asked for a better team backing me!
Experienced NDIS support coordinators committed to guiding you with clarity, care, and confidence.

To access support coordination in Newcastle, funding must be included in your NDIS plan under Capacity Building, Coordination of Supports. If it is already funded, you can choose your own support coordinator and are not limited to one provider.
If you are unsure whether it is included, your planner or Local Area Coordinator can clarify this at your next review. You are also able to change support coordinator at any time if your current arrangement is not meeting your needs.
To get started with Sunflower, you can make a referral directly through our website."
The level of support coordination depends on your circumstances and complexity of need. Level 2 support coordination is suitable for participants who require structured guidance to implement their NDIS plan and connect with services.
Specialist support coordination, often referred to as Level 3, is designed for participants with more complex situations, higher risks, or multiple service providers requiring careful oversight and collaboration.
Yes. An experienced support coordinator can assist with planning transitions from hospital, family care, or temporary accommodation into more stable arrangements.
This may include liaising with allied health professionals, housing providers, and local services to ensure supports are in place before any move takes place.
If you're exploring housing options, our SIL and ILO services may also be relevant to your situation.
The frequency of contact depends on your NDIS funding and individual needs. Some participants require regular monthly check-ins, while others may need more intensive coordination during periods of change or complexity.
Your coordination of supports should always reflect your goals, circumstances, and the level of oversight required.
Yes. Support coordination is intended to be independent and participant-focused. The role of a support coordinator is to connect you with services that best meet your needs, not to limit your choice.
This independence helps ensure provider recommendations are objective and aligned with your NDIS plan and long-term outcomes.
